Feedback, Compliments or Complaints
We encourage you to contact us with any feedback on your experience with the Radio Doctor service.
If you have specific feedback, suggestions or complaints we advise you to send details in writing / via email.
– your full name (or name of patient)
– your date of birth
– date & time of receiving the service
– name of the attending doctor
and send in complete confidence to:
firstname.lastname@example.org or to the above postal address and we will investigate and aim to respond within 10 working days. If you wish to make an offical complaint, you‘re entitled to do so by contacting the Health Complaints Comission on 180043159.
Privacy and Confidentiality:
We abide by various National and State laws and principles including the National Privacy Act (1988) and Health Records and Information Privacy Act 2002 (NSW) which define the standards by which this practice handles personal information collected from our patients.
